Passport Applications Accepted at:

City Clerk's Office
City Hall West (lower level)
300 LaPorte Avenue (map)

Regular Hours: Monday through Friday, 8:00 a.m. - 4:30 p.m. (except holidays)

Requirements:

Application Form
Be prepared to provide the following information:
- Social Security Number (Why is this required?)
- Occupation/Employer
- Information about your parents, including their place and date of birth
- An emergency contact person (someone who will not be traveling with you), including address, phone, e-mail address (optional), and relationship

Children under 16 -- refer to Special Requirements for Children Under 16 at travel.state.gov
Proof of U.S. Citizenship (No Photocopies)
- Certified Birth Certificate (available at: Vitalchek.com)
- Naturalization Certificate
- Previous U.S. Passport

Proof of Identity (must contain signature and physical description or photograph)
- Valid Drivers License (not temporary or learner's permit)
- Certificate of Naturalization
- Certificate of Citizenship
- Previous U.S. Passport
- Valid Government or Military ID

Two identical color photographs: 2" x 2" (head only) taken within last 6 months
- Passport photo service is available in the City Clerk's Office (Cost: $10 for 2 photos)
Fees (non-refundable):
- $25.00 payable to "City of Fort Collins"
  and
- $75.00 payable to "U.S. Department of State" (age 16 and older)
  or
- $60.00 payable to "U.S. Department of State" (under age 16)
- Additional fees apply for expedited service
Please Note: The U.S. Department of State and the City of Fort Collins
accept checks (with preprinted account name and address -- no counter checks) or money orders only. Credit/Debit Cards are not accepted.
