The City Clerk's Office has been designated as a Passport Application Acceptance Facility for the United States Department of State.
Hours: Monday through Friday, 8:00 a.m. - 4:30 p.m. (except holidays)
Requirements:
- Application Form
Be prepared to provide the following information:
- Social Security Number (Why is this required?)
- Occupation/Employer
- Information about your parents, including their place and date of birth
- An ememergency contact person (someone who will not be traveling with you), including address, phone, e-mail address (optional), and relationship
- Proof of U.S. Citizenship (No Photocopies)
- Certified Birth Certificate (available at: Vitalchek.com)
- Naturalization Certificate
- Previous U.S. Passport
- Proof of Identity (must contain signature and physical description or photograph)
- Valid Drivers License (not temporary or learner's permit)
- Certificate of Naturalization
- Certificate of Citizenship
- Previous U.S. Passport
- Valid Government or Military ID
- Two identical color photographs: 2" x 2" (head only) taken within last 6 months
- Passport photo service is available in the City Clerk's Office (Cost: $10 for 2 photos)
- Fees (non-refundable):
- $25.00 payable to "City of Fort Collins"
and
- $75.00 payable to "U.S. Department of State" (age 16 and older)
or
- $60.00 payable to "U.S. Department of State" (under age 16)
- Additional fees apply for expedited service
Please Note: The U.S. Department of State and the City of Fort Collins
accept checks or money orders only. Credit Cards are not accepted.
