Please feel free to print out any of these forms for your information. The Field Request form should be used to reserve fields for games, and can be faxed to the Parks department at 221-6849 or e-mail to This will not guarantee the reservation as dates requested may already be reserved. Fields do not have to be reserved for practice; everyone is welcome to drop-in and use the fields for practice, if the fields are open for use. However,if they have been prepped, this would indicate that a game is scheduled on that field and we would ask that you not use this field for your practice. If you would like to have your practice space guaranteed for your team, fields can be reserved for practice; scheduled practice times are: 3:30pm-5:00pm, 5:00pm-6:30pm and 6:30pm-8:00pm. If you have dropped-in to a field and another team shows up with a confirmation showing the field is reserved for their team, you must leave the field.
