MS Word If your computer has Microsoft Word, download this version and you can directly type in the form. If the file does not display in browser correctly, please download FIRST, then fill out. To download, right click the icon, choose "Save Target As..."

Get Adobe Reader If you do not have MS Word, download the Adobe Acrobat version. This will allow you to print out the application and fill it out using a typewriter or by hand. You do need the free Adobe Acrobat Reader installed on your computer to do this.
